Information provided in the Patient Medical Form enables us to determine an accurate diagnosis of your current health problems, as well as providing us with an overview of your medical history.

We will only record your email address if you send us a message requesting to join our mailing lists or register to receive our marketing material. In accordance with the Spam Act 2003, your email address will only be used for the purpose for which you have provided it unless you agree otherwise and, unless legally required to do so, we will not disclose it without your consent. You may unsubscribe to our mailing list at any time by emailing us with "unsubscribe" as the email title. Each e-newsletter provides clear instructions on how to unsubscribe.
